When the author saw a 721K program with this functionality, he figured he could do better in AppleScript.
Extension Adder adds an extension from a list of extensions or you can enter your own.

Optionally, you can shorten the existing filenames (to remove existing extensions). The script demonstrates the use of the "choose from list" OSAX (Standard Additions) and a bit of text manipulation.
